PHOTOGRAPHIC INDIVIDUAL CONSENT FORM
Melbourne Disc Golf Club wishes to record images of competitors at Social days, tournaments and special events for promotional purposes. These images may be used in mediums including publications and promotional material, broadcast, print and electronic media.
Melbourne Disc Golf Club asks individuals and/or parents and guardians to provide written permission for the use of these images in the form below.
Melbourne Disc Golf Club agrees not to use any photo in a manner that may be deemed adverse or defamatory to the person signing this form. The photo will remain the property of the Club and any personal details regarding this photo will be kept confidential and will not be used for any purpose other than related to this photo.
